Registrants should bring all their materials to the meeting. Forms received by mail and online after October 20, 2021, will be processed, but materials must be picked up on-site. They will not be mailed. Please check your packet for accuracy. Notify Mid-Continent Dental Congress immediately of any discrepancy at gslds@prereg.net. Avoid the Lines: Register Online! Register at mcdcstl.org to secure a spot in your preferred lecture, participation course or special event, based on availability. A confirmation email and receipt will be sent upon completion of your registration. Mailed forms will be processed in the order received. Registrations cannot be accepted by fax or phone. Mid-Continent Dental Congress accepts Visa, Mastercard, American Express or check (payable to Greater St. Louis Dental Society). A $3.50 transaction fee applies to all credit card registrations. Cancellation Policy Cancellation requests must be sent via email to gslds@prereg.net by October 7, 2021. All approved refunds are subject to a $25 processing fee. No refunds will be made for cancellations requested after October 7, 2021. All approved refunds will be returned in the original form of payment. If registration materials and badges have been received, you must return all materials before a refund will be processed to: Mid-Continent Dental Congress c/o Eleventh and Gather 6840 Meadowridge Court Alpharetta, GA 30005 The Greater St. Louis Dental Society Board of Directors and Council on Scientific Sessions will continue to carefully monitor the COVID-19 situation and its potential impact on the Mid-Continent Dental Congress (MCDC). In the event the convention may not be held as scheduled due to a state or local government mandate, all registered attendees will be refunded in full. Badge Will Serve as Your Ticket(s) The backside of attendee badges will list events for which the attendee has registered, and will serve as admittance into all events. On-site Event Transfer Policy Once a course or event has begun, no transfers will be allowed. Refunds will not be available for any on-site changes. Lost badges Badges must be worn at all times during the meeting. Courses for which the attendee has registered will be listed on the backside of their badge. Attendees will not be able to enter any course for which they have not registered. A replacement fee of $10 will be charged for lost, stolen or misplaced badges. COVID-19 The GSLDS Board of Directors and the Council on Scientific Sessions continue to closely monitor the ongoing impact of Covid-19. As the situation evolves, so will our approach to keeping you safe. Our safety protocols will follow the current recommendations from the CDC and local public health authorities in place at the time of the event. All registered attendees will be provided with details regarding safety measures in their registration packets. Updated information will also be available on mcdcstl.org.
